The Heart Failure is for Life SM Award goes to those individuals with diagnosed heart failure that are stand out examples to others of the indominitable human spirit and a reminder that you are not alone.
The Colloquium’s Heart Failure is for LifeSM Award Committee considers the following five areas of when reviewing nominations:
- Strength of the overall story
- Relationships the individual has to family
- Relationships the individual has to community
- Service to others despite their own heart failure
- Individual special circumstances or human interest aspect
